Getting Started:
Problem Identification
Each patient must know what the problems are before beginning
to address them
In the first sessions with your primary counselor (Case Manager)
you will discuss the pertinent problems that are impacting your
life and which have brought you into treatment. Then you will
begin to explore and identify the significant underlying issues
that strengthen and perpetuate these problems.
This collaborative process will result in a very specific
Problem List – which may be expanded as you learn more about
yourself, your condition and your behaviors. (Ongoing
Educational sessions are designed to assist with this).
The most significant of the identified problems will become the
focus for your individualized treatment.
Setting the Course:
Treatment Planning
The Road Maps for treatment; Care Plans provide the
steps to reach your identified goals
For every problem there is a solution; arriving at the solution
requires strategic planning, direction and clearly defined
objectives. With your input, your Case Manager will develop the
Treatment Care Plans to guide you toward both short- and
long-term goals in recovery. In this process your Case Manager
will consider your: History and experience with treatment and/or
recovery; Willingness to face the issues; Level of acceptance of
your condition and situations; Current level of support from
family and others; as well as what you want for yourself in your
recovery.
Depending on your level of awareness and acceptance of your
condition (addiction), Care Plans can range from Discovery of
a disorder to Recovery from a disorder.
